Insurance Enrollment instruction for newly eligible employees:

Enrollment will be done over the internet.

There are 2 different web sites you will be going to when you are finished with the first one it will take you automatically to the second one. The first web site will allow you to enroll in Medical, Dental, Free Life and Additional Life. The second one will allow you to enroll in Short Term Disability, Long Term Disability, Accident Coverage, and the Flexible spending for Medical and Daycare.


Medical, Dental, Free Life and Additional Life

You will need to go to

  • Your USER ID is your 6 digit payroll number (example payroll #4069 you will enter 004069).
  • Your PASSWORD will be your SS# (does not matter if you use dashes or not).

This will then bring up your personal information that you have to update. You will need to update your dependent (children) information first; this does not automatically cover them on insurance you will elect that after all personal information is updated.

After updating all personal information it will then take you to a screen that gives you info on the benefits that are available to you and the cost.

You will then proceed to accepting or declining coverage's. You will then be prompted to continue to the applications and beneficiary information at that time for the coverage's that you have chosen.

When you are finished and have printed what you have chosen you will then click on the final step and it will take you to the American Fidelity web site to finish.

You will need to print the American Fidelity instructions to help you with this web site.

Open Enrollment For Your Supplemental Benefits

Accessing the Enrollment Site: When you finish on "See Benefits" there will be a link to direct you to American Fidelity website to finish your enrollment. Or access the enrollment site at Click on the "Online Enrollment Services" link to access the secure enrollment site. On this page you will find:

Group Login:

In the Group Login box type in the group login name (case-sensitive) and click the "Continue" button to proceed on to the next screen. Contact your Regional or District Manager if you've forgotten the group login name.

Individual Login:

First Time Access: If this is your first time to access this page click on the "Register Here" button.

  1. Register: On the next screen, complete all of the required fields. Then click the "Create Account" button. Carefully read the instructions on the next screen, you will need this information to successfully login to your new account.
  2. Login: The next step will be to login to your newly created account. Follow the instructions given on the next screens to gain access to the enrollment system.

Already Registered: If you have already registered, you will provide the following:

  1. Login: Enter your Social Security Number without any dashes or spaces. Then press the Tab Key.
  2. Password: Enter the password you created during the initial login procedure.
  3. After completing the required fields click on the "Enter" button to proceed.

CONTINUES: for up to 366 Days

Long Term Disability Benefits are Payable Beginning:

(Must be purchased with the Short-Term Plan)
ACCIDENT: On the 366th day of Total Disability due to a covered Accident
ILLNESS: On the 366th day of Total Disability due to a covered Illness
CONTINUES: until age 65 for an accident or up to 5 years for an Illness

Some good news about taxes...

You don't often see the words "good news' and "taxes" in the same sentence. But Section 125 of the Tax Code allows your out-of-pocket medical expenses and dependent day care expenses to be paid for with pre-tax dollars through the Flexible Spending Accounts. That means that you are taxed on less income, so you take home more or you have more dollars available to purchase other benefits. That's good news!

Flexible Spending Accounts

A Flexible Spending Account is a great way to pay for out-of-pocket Medical Expenses and/or Daycare Costs. When you enroll in a Flexible Spending Account, you will be provided with a voucher packet. Each pay period, the amount you designated will be deducted from your paycheck and deposited into this account. To receive reimbursement, you will simply need to complete and submit a claims voucher from your packet.

Flexible Spending Accounts allow you to save money because deductions are made from your paycheck before taxes are held. And because you know your daycare expenses and out-of-pocket medical expenses, you determine the amount that you contribute to this account. However, any money remaining in your Flexible Spending Accounts at the end of the year will be forfeited, so it's important to carefully estimate your needs.

Medical Expense Reimbursement

You can contribute up to $5,000 per year to pay for eligible medical expenses.

Dependent Day Care Reimbursement

You can contribute up to $5,000 per year to pay for eligible dependent day care expenses. For more details about these two plans, login to the enrollment web site using the instructions provided above.

Accident Protection PlusTM

Accident Protection... For You And Your Loved Ones. Accident Insurance coverage provides protection for you and your family against the additional expenses associated with accidental injury. You can never plan for an accident to happen, but you can plan your protection against the high costs associated with accidental injury and death. This plan provides Accidental Death and Dismemberment and medical expense benefits due to injuries sustained in a Covered Accident. Benefits are payable from the 1st day of coverage and are paid regardless of any other coverage you might have.
